New and Amended Approved Forms

The Rule Committee has approved three new forms for immediate use:
- Form 14A - Application for Industrial Instrument (PDF, 137.8 KB)
- Form 28 - Application for Leave to Appeal and Appeal (PDF, 148.2 KB)
- Form 28A - Notice of Contention (PDF, 114.7 KB)
These forms are available in the Approved Forms section on the Commission's website from today.
Applications for new and varied awards made using Form 1, Application for Approval of Enterprise Agreements, using the existing Form 14, and Applications for Leave to Appeal and Appeal commenced using the existing Form 28 will be accepted until 28 February 2023.
