New Online Registry forms coming soon

The Industrial Relations Commission is currently developing new forms that will soon be available on the Online Registry Website:

  • Application to Commence Proceedings for a Summary Offence
  • Application for Relief from Victimisation
  • Application in Respect of an Industrial Instrument
  • Application General Form
  • Summons to Produce Items Before Hearing
  • Summons to Produce Items at Hearing
  • Summons to Attend to Give Evidence
  • Summons to Attend to Give Evidence and Produce
  • Points of Claim
  • Statement of Facts
  • Witness Statement
  • Notice of Motion (Civil only)
  • Notice of Motion – Substituted Service (Civil only)
  • Notice of Appearance (Crime only)
  • Notice of Change of Solicitor (Crime only)
  • Notice of Ceasing to Act (Crime only)
  • Notice of Intention to file Notice of Ceasing to Act

The addition of these Forms will make it easier for parties to file their material with the Industrial Court and the Commission.  To do so, users must register in the Online Registry portal and create an account.  Given these forms can be filed using the Online Registry they are not to be filed by email, unless technical difficulties prevent you using the Online Registry. 

PLEASE NOTE that any ‘Application to Commence Proceedings for a Summary Offence’ filed after 4pm on any business day will not be considered by a Judge before the following business day.
